When you accept the appraisal output Form, the appraisal feedback questionnaire will be displayed automatically (if this option is enabled for your organisation). Only upon completion of this form will this appraisal be completed and a new appraisal opened.

The content of the completed appraisal Feedback questionnaire is available to the head of appraisals, but your name is not included. Your Appraiser will not have access to the completed questionnaire.

When you have completed the appraisal feedback questionnaire, your current appraisal is closed and a new one will be created and opened automatically.
